Cancellation Policy:
ORAL CANCELLATION WILL NOT BE VALID. Member may cancel this agreement by giving a written 30-day notice. Monthly Memberships will continue until receipt of a written 30-day notice requesting to terminate agreement. Cancellation notice will not be accepted until all money owed to PUSH FITNESS has been collected. Member will receive NO refunds for services unused or for lack of attendance by member. Member agrees to pay the remaining balance of this contract if PUSH FITNESS is unable to charge account during any month. If any banking information changes, member will update account to avoid additional fees. Past monthly dues, late fees and all outstanding balances will be due immediately before this contract is terminated. Notice of cancellation is considered given by mail when postmarked. Member indemnifies and holds PUSH FITNESS harmless, against any liability pursuant to this authorization and if this account is ever past due and subsequently turned over to a collection service, member agrees to pay all collection fees in addition to obligations to PUSH FITNESS. Member understands that signature on this Membership Agreement will serve as authorization for these charges. Member agrees to the terms and conditions as outlined in this statement and that all sales are final with no refunds or exchanges.
